Hi, please try this:

1) Open Customize->Customize User Interface->Toolbars.
2) Select "RailClone" toolbar in the right combobox.
3) Click Delete it to remove it.
4) Restart Max.

Hopefully that should reset the toolbar and restore the missing icon.
