Every time 3dsmax is restarted, the quad menu will be added automatically

I don't know why you have duplicated entries. We cannot reproduce that here.

Please, try at Customize->Customize User Interface->Quads->click first quad box (tools1), delete all "RC_MS_" entries and restart Max.
If menu duplicates again, please tell me.
